Thou shall not be late or unnecessarily early for thy appointment.
If you arrive more than 10 mins late your session may be shortened. Drive safely and notify me directly via text or call if you are behind, I will do the same.
Thou shall not cancel or reschedule an appointment with out a notice of 24 hours.
If 24 hours notice is not provided a cancellation fee of 25$ may be charged or deducted from your gift certificate. If I am able to fill your appointment or you are able to find someone to come to the appointment in your place, the fee will be waived.
Thou shall not forsake thy appointments or cancel appointment with in 8 hours of the scheduled time.
If you no show or cancel an appointment with 8 hours of the scheduled time, you will be charged for entire session.
Thou shall keep thy medical information up to date.
It is imperative that you notify me of any changes to your health, allergies, injuries or medication.
Thou shall keep track of thy gift certificates.
Gift certificates have no cash value and can not be redeemed if lost or stolen.
All thy medical information is kept completely confidential
I do not share, disclose or grant anyone access to your protected medical information with out your written request to do so.
